Visiting Our Clinic

New Patients

If you are coming to our office for the first time or are interested in joining our practice, filling out the new patient form and medical records transfer form (if needed) will make your first visit quicker and easier. Click the button below or go to the "new patient" page for more information.

All (including returning) patients

If you are a current patient of our office, please remember procedures for appointments.

  • please show up 5-10 minutes before your appointment time; please factor in traffic, construction, weather; constraints imposed by school, work, etc. schedules. We are a pediatrics (ONLY) office and many of these issues are in common with families. We depend on punctuality to permit a continuous flow in clinic and maximize our access. 
  • No shows (cancellations < 24 hours without emergency, missed appointments without communication) may incur a fee before rebooking; repeated no-shows may limit/prevent further bookings.
  • if you are coming in to review results, please  ensure we have  the results . You may  call prior  to see if we have received them or contact the sending facility.
  • If your concerns involve a review or change of medications, please ensure to have salient details about your prior medications PRIOR to the appointment. Your may need to review with your pharmacist as well if required.
  • we book per patient not per family; if you would like another child of yours to be seen at the time of a your current child's appointment, please call us ahead of time so that we can schedule accordingly
  • make sure your child's health card is valid (a $50 deposit is required if card is expired)


Results Procedure

NO Walk-In Patients

Due to COVID pandemic  we are NOT accepting walk-ins.  Please call to schedule your appointment to help us plan flow of patients. Same day appt maybe available but there would be very limited availability.
