Dr. Lau and Dr. Jacob are accepting new patients (by doctor's or midwife referral in some cases). If you would like to become a new patient and are currently a patient of another office, send or give your signed completed medical records transfer form to your doctor's ( a fee may be charged by their office), and ask to have your child's record transferred .  All new patients can fill out the "new patient form" below and bring it on your first visit.
